What if I cannot get documentation?

For extenuating circumstances, Academic Advisors can grant accommodations based on compassionate grounds. For more information, arrange an appointment with an Academic Advisor here

What is the process for receiving academic accommodation?

Students should complete the Request for Academic Accomodation form and send this form, along with their Student Medical Certificate (if approporiate) to brescia@uwo.ca.  An advisor will receive and process the request. The student will then get an email confirmation from and advisor. At this time, the student is responsible for arranging accommodation with their professor.

How do I receive an accommodation for a final exam?

There are various reasons for final exam accommodation including Personal, Compassionate, multiple exams or time conflict. For more information, visit Western's Exam Schedule.

Students are required to send valid documentation to brescia@uwo.ca. For final exams, students will need to fill out a Special Exam form. Students are responsible for acquiring any signatures needed to complete the form.

What is the process for arranging for accommodations with Student Accessibility Services?

Students will need to forward their Academic Accommodation Form as well as the Accommodated Exams Request Form. Students are responsible for ensuring their instructor and academic advisor sign off on the letter before submitting it to the Student Accessibility Services Office. Please note: This process is for accommodations only. Students need to arrange the initial exam/test with the Student Accessibility Services Office.

How do I request academic consideration?

There are three ways to request academic consideration:
1. Self-Report Absence
2. Medical Absence
3. Non-Medical Absence

What can be self-reported?

2. Midterm exams scheduled during the December exam period
3. Final lab exams scheduled during the last week of the term

How many times can I self-report?

Students are able to submit a maximum of two self-reported during the academic year (September - April) and a maximum of one self-reported absence during the summer term (May - August).

How do I self-report?

1. Log into Student Centre
2. Click the Self-Report Absence link located on the right-hand side under Student Services links
3. Carefully read the instructions and complete the form, indicating each of the courses that you will miss during your absence
4. Email your instructor(s) following your self-reported absence to dicuss a new deadline/make-up date
